I tried to do a little research. It could possible be from a series called 'lucky louie' which has some male nudity.
If you look at the cast at

The first black lady looks like possibly kim hawthorne playing "ellen", and the second brunette could possibly be either Pamela S. Adlon playing "kim" or Laura Kightlinger playing "Tina".
I am not a 100% sure as I have never seen this series, but I have a feeling I could be on a winner. I need confirmation from someone.

This is an incredible clip. For those into small penis humiliation, it really brings it out of the closet in a big way. The filming style and accompanying laugh track makes it appear to be a mainstream sitcom, although it of course could not be.
Regardless of it being a comedy, and that he's an actor there to get laughs, you cannot deny the fact that he is still a real man that truly has a tiiny tiny penis. He then presents his little dick to the world to see as well as presenting it to these two actresses so they can see it, point at it and laugh making jokes about it. I hope it isn't too good to be true. Is there any evidence that anything has been doctored with here? The whole thing seems to fall into place quite well though. I sure wish I could see the rest of it.

This area of CFNM is very controversial. There are many that don't agree that CFNM can include flavors of BDSM or humiliation. I have always disagreed. Both white and red are wines, you choose what you like.
As it pertains to this clip, I cannot imagine it is doctored (no pun intended) in anyway, and if it is, such doctoring would have to be a professional job. It's one thing to doctor a photo with Photoshop, it's another thing to doctor hundreds of still captures - costly and very time consuming.
I also highly doubt that it is an amateur production. We've seen too many amateur and low-budget CFNM productions to know that the quality of this one is far more professional. They are real actors with scripts and execute them in a way you'd expect to see of an "HBO" type of series, not some bimbos that answered a Craigslist ad. And the set for the production required significant professional work - if you look at amateur or porno production of doctor office scenes with nurses, they typically have a blank wall with one (1) anatomy poster, a chair or gurney, and, maybe a counter... that's about it. This one is too realistic for it to be an amateur or small time porno production - I doubt they shot this in a real hospital or doctors office either. The set looks exactly like a doctor's office or hospital room would look like...so a lot of work and expense went into set design and creation.
I *really* think this is exactly what it is meant to be - a scene where a real actor who has a very small penis is naked with his dick on display for all to see while these two women look at it and laugh at it make jokes. And it is clear that the intent of the scene was such that the audience can both see his little dick while enjoying seeing him being humiliated by these two women for having it.
It sort of shows that small dick humiliation with real CFNM staging could also be enjoyed in a more mainstream way than just this board.

I managed to see the full episode (episode 5).
The scene starts with the doctor being a replacement for his regular doctor. He is reluctant to move his hands out of the way. At that point the nurse comes out, as in the video above. After that the nurse puts him in a full nelson while the doctor takes pictures (I was right about the camera). One thing to note is that after watching the full video it becomes obvious that the actor is actually wearing some kind of cup with a prosthetic.
If I can find a way to cap it and share it, I will.

Perhaps it is a prosthetic. I do know that they have nude men in Zumanity (Cirque du Soleil in Vegas at NY, NY) that have prostetics to imitate the real thing...never seen it though, so don't know.
